How should campaigns be separated?

By anything you need to control independently. Different services with different values, different geographies, different margins — each of those is a reason to separate, because separation is the only way to give them different budgets.

The opposite failure is over-separation: dozens of tiny campaigns each generating too little data for bidding to work with. The balance depends on volume, which is why there is no universal template.
